Registration information

BACKGROUND:
Aniridia is a genetic eye disease / birth defect made up of conditions such as glaucoma, cataracts, corneal scarring, low vision, blindness, and in some children diabetes, learning disabilities, Wlims tumor (a canerous tumor of the kidney before age 8 yrs old) and retardation. Although these conditions are commonly found in the general population, It is uncommon to be born without developed eye structures such as the optic nerve, retina or iris, and to have all or most of the conditions listed above in one child. It is a definite challenge to live for the rest of their lives with these issues.

Bowl for Childhood Blindness is an annual event and is one of the two charity events going on Saturday, Aug 1, 2009. (Golf for Childhood Blindness scramble is the other). Registration costs of this event will include 3 games of bowling, shoes, prizes, followed by the "Make a Miracle" charity dinner and an evening of fun, raffles, silent auction items and music.

After bowling, we will all meet back at the Hilton Indian Lakes Resort (1 mile away) where we will join the participants of the GOLF for Childhood Blindness event for our cocktail hour / charity dinner. AFI hotel guests without cars will be shuttled to and from Brunswick Zone in Roselle, IL.

Prizes:
We will be awarding some prizes for best team name, best team themed outfit, high series in group and others. Be creative and have fun.

REGISTRATION:
Please fill out the form completely, pay with your credit card, print out your receipt and bring it to the event as your ticket. Your name(s) will be on the sign in sheet and we will call you to let you know that you have been registered so please make sure your phone number is correct. If you are registering as a single person and need to be matched up, list the 2nd team member as "NEED A PARTNER".

If you are a REGISTERED AFI conference attendee, your rate is reduced since the dinner was figured into your conference registration fees. Make sure you list your full names under TEAMS for verification.

Teams:
This will be a Scotch Doubles format. You can register 2 person teams or mark below that you need to be matched up with someone. Also, sign up under the designated group for each team.
Groups available:

Parent / Child Team: (child under 12 years or younger)

Children will throw the first ball; Parents throw the 2nd or pickup ball

Teens or Adult Team: (regular Scotch doubles)

One team member throws the first ball and other team member the 2nd ball (switch each game)
